Please include your name, partners name,Team Number, contact phone numbers, and a check for $80.00, made
payable to Ben Carter, 4637 Flatwood Lane, Orlando Fl, 32829.....DO NOT make check payable to "Courtesy Anglers"... This is for returning
members with the same partner ONLY!! Fee's are $40.00 per team member,$80.00 per boat, and includes T-Shirt 5.0 program.
Please include you and partners T-Shirt size with registration info. Payment for BOTH members must be included at time of mailing.
